How To Enter The Sweepstakes
There are four ways to enter the CareCredit Sweepstakes.
- Earn one entry for making a qualified transaction made on your CareCredit credit card or CareCredit Rewards Mastercard credit card during the Promotion Period.
- Earn one entry into the Sweepstakes when you sign up for e-Bill for an Eligible Credit Card during the Promotion Period.
- Earn an additional Sweepstakes entry by playing the Instant Win Game and receiving a message that you have earned an additional Sweepstakes entry.
- Receive one entry without making a qualified transaction, signing up for e-Bill, or if you are already signed up for e-Bill prior to the Promotion Period, hand print your name, complete mailing address, telephone number, email address and date of birth on a plain 3″x5″ piece of paper and mail in a hand-addressed stamped envelope to: CareCredit 30 Years of Caring Sweepstakes Entry, P.O. Box 7729, Melville, NY 11775-7729.

Sweepstakes Grand Prize (1): $30,000, awarded in the form of a check.
Sweepstakes First Prizes (30): $200, awarded as a credit to the winner’s CareCredit or CareCredit Rewards Mastercard credit card account and a $100 Sponsor-specified CharityChoice gift card.

Eligibility:
This Care Credit Sweepstakes is open to legal residents of the U.S., including its territories and Puerto Rico, who are 18 years of age or older as of date of entry/Game Play, who are cardholders of a CareCredit credit card or CareCredit Rewards Mastercard credit card (each, as of May 14, 2017 and on the date of entry/Game Play with an account in good standing as of date of entry/Game Play and prize award date.
Sweepstakes Period:
The Promotion begins 12:00:00 AM ET on May 15, 2017 and ends 11:59:59 PM ET on June 30, 2017.
Limit Of Entry:
Limit 4 Sweepstakes Entries per person/Eligible Cardholder throughout the Promotion Period, regardless of method of entry or combination of methods of entry.
Limit one Game Play per person/e-mail address/Eligible Credit Cardholder per day.
Drawing Dates:
The Grand Prize and First Prize winners in the Sweepstakes will be selected in a random drawing from among all eligible Sweepstakes entries received and postmarked during the Promotion Period. The random drawing will be conducted on or about July 26, 2017.
